Email 198: On PhilHealth and Switching Hubs
Posted by Pozziepinoy on 3:44 PM
Dear Pozzie,
Good afternoon, I have a question regarding PhilHealth. My doctor asked me to have my CF1 forms signed by my employer. Now the problem is the person handling PhilHealth in our office is asking why I'm asking for a claim when I'm not yet in the hospital.
My question is...is it necessary to have it signed by my employer? I'm new in my office and they are making some adjustments on my records and it took me days until now waiting for the forms to be signed. I rescheduled my last counselling (eventually my start of ARV), my doctor schedule me this coming Monday. What will I do? I want to start my treatment so I can adjust on my treatment. Plus, I want to transfer to another hub, preferably PGH-SAGIP (I wanted sana RITM-ARG, kaso it's far from my place if testing will be needed).
What would be my next step? Parang I'm not comfortable with my hub right now even though it's really accessible to my house. I really need an advice. I'm just starting pa lang. I was able to tell it to my cousin and he understands and gave comfort ,which I thank God and you for telling me that confiding it with your family will help me bounce for recovery.
Please I need your advice...Thank you and God bless....

CONTRIBUTOR'S RESPONSE
Hello,
Thank you for writing an email.
I am happy that you have your cousin who understands and gives you comfort. For employed individuals it is necessary that your CF1 is signed by the employer, in your case you can maybe come up with an alibi, for example one of your beneficiaries may need your PhilHealth contribution for check up or laboratory test. If that won't work, what other pozzies do is they apply for a different PhilHealth under voluntary contribution so they don't need the signature of their employer.
For your second question, you can call S.A.G.I.P at 5548400 loc 3249 to inquire about how to transfer hubs. Hopefully things will be better for you.
Please feel free to write again.
